The tournament you are requesting to attend MUST NOT interfere with regular league or provincial play.
If a game is to be re-scheduled, it must be agreed upon prior to submitting this tournament request.
You may not attend a tournament without Approval of the HMHA Board of Executives.
This tournament request must be submitted minimum 2 weeks prior to start date of the tournament.
If you are NOVICE or INITIATION level, you must obtain a Travel Permit.
If you are Atom and above you will require a Travel Permit if you are leaving zone 3.
